Open communication with your health care provider ensures you receive the very best possible care. Follow these tips during your next appointment:
Bring a complete list of your medications, including over-the-counter drugs, vitamins and herbal supplements, along with their dosages and directions.
Have a list of health concerns you’d like to discuss with your provider. Review your questions for your provider and ensure your top three concerns are addressed during your visit.
Bring details of any health care providers you’ve previously seen, including names, dates of treatment and contact information.
Please arrive 30 minutes early to ensure timely check-in and no more than 10 minutes late to help minimize wait times. We understand emergencies may arise and will accommodate them when possible.
Only urgent issues will be addressed after hours. For non-emergent concerns, please schedule an appointment. Prescription refills are limited outside regular office hours.
Don’t forget to schedule your follow-up appointment before leaving the office to ensure timely and consistent care.
If you have pending lab or test results, there's no need to call our office unless specifically instructed by your provider. Once reviewed, results will be available on your patient portal. If your physician deems it necessary to discuss the results, they or a certified medical assistant will contact you with recommendations. Please ensure we have your up-to-date phone number on file to reach you.
We’re here to make your visit as simple and efficient as possible. With mobile check-in, you can securely register from the comfort of your home using your smartphone, tablet or computer. Ask us about mobile registration for a quicker, smoother experience.
Our CHI Memorial Answering Service handles calls from 4 p.m. to 8 a.m. on weekdays and throughout the weekend. For urgent matters that cannot wait until office hours, the service can connect you with the on-call provider. Please use this service for after-hours care.

We understand that visiting the clinic, leaving messages, waiting on hold or updating personal health information isn’t always convenient. That’s why we’re proud to offer secure online access to your health information, making it easier and more convenient to manage your care.
We accept most major insurance plans. All insurance copays, coinsurance and deductibles are due at the time of service. Please be prepared to settle these before you leave the office. We are committed to keeping costs low while providing high-quality care. To ensure accurate billing and timely communication, please inform our receptionist of any changes to your address, phone number or insurance information. This helps us file claims correctly and avoid unnecessary billing issues.

We value your time and do everything possible to minimize your wait. If you're unable to keep an appointment, we kindly ask for a cancellation call at least 24 hours in advance. This allows us to offer your time slot to another patient in need. Appointments not canceled within 24 hours may incur a fee. Our goal is to make the best use of our providers' time to help as many patients as possible. Repeated missed appointments could lead to dismissal from our practice.
