New patients will need to arrive 30 minutes early, and established patients 15 minutes early, to their appointment. This gives us time to enter your information in the system and have you ready for your appointment on time. Follow these helpful tips to ensure a positive experience:
To ensure a smooth check-in process, please arrive 15–30 minutes early. New patients should arrive 30 minutes before their appointment to allow time for us to enter your information into our system. If you’ve preregistered using Phreesia, you may arrive 10 minutes early instead.
Bring your insurance cards, driver’s license and any documentation proof of changes to your legal name. Make sure your email, phone number and emergency contact information are up to date. to your appointment. This information is used for appointment reminders, lab results and patient portal invitations. If you’ve already provided this information during preregistration, you won’t need to provide it again at your appointment.
Review your list of questions with your provider. This will help ensure your health concerns are addressed during your visit.
Please inform your provider about any medications requiring refills during your scheduled appointment. If you request a refill by phone, we’ll send it electronically to your preferred pharmacy.
Bring legal or other necessary forms, such as prior authorization, FMLA, disability or medical supply forms, to your appointment. Please allow 72 hours after the appointment to complete the forms and arrange for pickup. If your prior authorization for a procedure or medication is denied, contact your insurance provider for further details.
Schedule your follow-up appointment before leaving the office to ensure timely and consistent care.
Normal lab results will be accessible through our patient portal. If you're not yet registered, please ask our receptionist for assistance. If any lab results are abnormal, our office will reach out to you by phone.

We understand that visiting the clinic, leaving messages, waiting on hold or updating personal health information isn’t always convenient. That’s why we’re proud to offer secure online access to your health information, making it easier and more convenient to manage your care.
We accept most major insurance plans. Check your policy to confirm coverage, so we can submit the claim correctly. All insurance copays, coinsurance and deductibles are due at the time of service. Some insurance plans cover annual physicals.

We respect your time and strive to keep wait times to a minimum. To assist us in providing timely care, please notify us at least 24 to 48 hours in advance if you need to cancel your appointment. Missed appointments without notice may result in a fee. This helps us optimize our providers' schedules and serve as many patients as possible.
