You have the right to:
Understand your rights before receiving or discontinuing care, and to file a grievance or complaint if needed
Make informed choices or designate someone to make decisions for you
Know your medical condition and be part of planning your treatment
If you are a Medicare patient, you can request a review of concerns regarding the quality of care or an early discharge, and ask for your concerns to be addressed through specific review processes. You also have the right to create Advance Directives, and ensure that our team follows them.
Your health care works best when you, your family and your care team work together. By following these simple responsibilities, you can help ensure that your care is as effective as possible:
Tell us about your health history, including any medications, vitamins or supplements you’re taking.
Follow the instructions of your care team carefully and ask your nurse or caregiver any questions about your care, safety or concerns.
Respect others and follow the hospital’s rules to help maintain a calm and safe environment.
Provide copies of any Advance Directives to the hospital so we can respect your wishes if you are unable to speak for yourself.
Pay your hospital bills on time, ask any questions you have about your bill and provide the necessary information for insurance processing.
