Our clinic specializes in follow-up care for patients recovering from a stroke or transient ischemic attack (TIA). We focus on assessing recovery, managing risk factors and creating personalized care plans to reduce the likelihood of future strokes.

Your care will be managed by a team of highly skilled providers, led by specialists with expertise in stroke and neurological care. Our team is dedicated to supporting your recovery and long-term health.

During your visit, we will review your medical history, evaluate your recovery progress and assess your risk for additional strokes or TIAs. We may recommend diagnostic tests, lifestyle adjustments, speech and occupational therapy or medications to manage risk factors.

Stroke rehabilitation often involves comprehensive care, including the proactive management of blood pressure and cholesterol, along with the use of aspirin or other medications as recommended. After hospital discharge, patients are typically referred to a neurologist for follow-up care to ensure all aspects of recovery are addressed. This care plan may include inpatient rehabilitation, home-based therapy or outpatient rehabilitation programs tailored to individual needs.

At CHI Memorial, our mission is to support patients throughout their entire journey from stroke to recovery. We are committed to providing seamless care and ensuring patients have access to the resources and support they need for a safe and successful transition from the hospital to home and beyond.
