About this location

Continuing Care Hospital (CCH) is a 25-bed long-term acute care hospital located inside Saint Joseph Hospital. CCH offers various services to manage complex medical needs. Our skilled and compassionate staff expertly addresses conditions requiring an extended hospital stay. Our facility features all private rooms and promotes patient and family interaction. Licensed as a general acute care hospital, CCH supports the needs of unique and complicated physical, emotional, spiritual, nursing and rehabilitative needs of our patients. Our goal is for you to return to your previous home environment.

Patients and visitors

At CCH, we offer exceptional hospital-level care for patients requiring an extended hospital stay. Our private rooms provide the necessary patient privacy, which helps reduce the risk of infection and promote patient and family interaction. We serve patients with heart conditions, complex medical and post-surgical conditions, prolonged mechanical ventilation, pulmonary disorders and wound care needs.

Our multidisciplinary approach includes a collaborative team of health care providers and staff, including a pharmacy and wound care specialist, dieticians, physical, occupational and speech therapists, nurses and case manager. Our chaplain is also available to help care for your spiritual needs.

Long-term acute care hospitals are a special classification of hospitals recognized by the federal government for meeting the required specifications. Long-term acute care hospitals are certified to care for patients requiring an extended hospital stay. The average length of stay for an acute-care patient is at least 25 days.

At CCH, we care for patients with:

  • Cardiac conditions

  • Complex medical conditions

  • Complex post-surgical conditions

  • Long-term IV antibiotic therapy

  • Prolonged mechanical ventilation and ventilation weaning

  • Pulmonary disorders

  • Wound care needs

We are very proud of our accomplishments with quality outcomes and standards using evidence based practice.

Our staff are experienced in caring for the medically complex. We’ve established several teams to assist in the prevention of falls, infections, pneumonia, pressure ulcers and deep vein thrombosis. We have also received several quality awards for the services that we provide at our facility.

Located on the ground floor, our cafeteria is open daily except weekends:

  • Breakfast: 6:30 to 10 a.m.

  • Lunch: 11 a.m. to 2 p.m.

We also have Starbucks located in the front lobby, open 7 days a week:

  • Monday – Friday: 7 a.m. to 7 p.m.

  • Saturday – Sunday: 7 a.m. to 3 p.m.

The gift shop is located on the first floor. Hours of operation are Monday – Friday, 9 a.m. to 5 p.m.

We provide free interpreter services for our limited English-speaking patients and related communication equipment and services for our hearing-impaired patients.

We offer a variety of resources to meet language needs, including:

  • Face-to-face interpreters for many languages, including Spanish and American Sign Language

  • Over-the-phone interpreters for most spoken languages

  • Video-remote interpreting for American Sign Language
