More about your medical records
Your medical records are a vital part of your health history and include details like medical history, diagnoses, lab results and reports from tests and procedures.
In most of our communities, you can use a free and secure online patient tool to request your records. Patients with a portal account can also access a portion of their records through their patient portal. However, some records may not be available online or may require an authorization form. Additional options to access or request your medical records are also available to meet your needs.
