Annual Wellness Visit

Annual wellness visits (AWVs) are critical pathway in identifying high risk patients and helping them to develop a personalized prevention plans to prevent illnesses, including immunizations, medication review, and advanced care planning. They can be a valuable tool to ensure appropriate quality gaps are closed.

Patients who have Medicare Part B are eligible to receive one annual wellness visit every 12 months, free of charge.

Developing and expanding initiatives to increase annual wellness visit awareness and participation among our Medicare population should lead to improved quality of life and lower health care costs for our elderly patients.

Tools and evidence-based strategies have been developed to facilitate our Accountable Care Organizations in the implementation and improvement of compliance with Annual Wellness Visits.
