Primary care providers in select pilot markets will be able to identify candidate patients for an evidence-based collaborative care model approach, shepherding patients through a care network and connecting them with treatment, from counseling to medication therapy.
A population health approach will also be used to proactively identify patients that would benefit from such an intervention. This sort of outreach, patient navigation, and collaborative care will assure that patients are receiving the care that they need.
It also helps share some of the treatment and referral responsibilities of primary care providers with mental health treatment specialists. Evidence has shown that such an approach works, and it helps save time and increase value, while all along promoting equitable access to high-quality mental health services.
