MyChart for hospital visits

MyChart is a secure online tool for patients of CHI Memorial Hospital Chattanooga, CHI Memorial Hospital Hixson and CHI Memorial Hospital Georgia. It lets you easily access your health information, test results and care plans from the comfort of your home.

How to create a MyChart account

• Go to mychart.memorial.org.

• Click “Sign up now” and enter your activation code and date of birth to confirm your identity. 

• Follow the on-screen instructions to set up your account.

• Log in with your username and password. 

You can also download the MyChart app from the Apple App Store and Google Play.

My OneCare for clinic visits

My OneCare, powered by FollowMyHealth, is an easy-to-use patient portal for anyone who has seen a CHI Memorial Medical Group provider. 

Getting started

After your appointment, you'll receive an invitation to join the portal at the email you provided. The email, sent from noreply@followmyhealth.com, will include sign-up instructions. If you don’t see it, be sure to check your spam folder.

Creating a FollowMyHealth account 

• Click the registration link in the email from noreply@followmyhealth.com. 

• Create a username and password.

• Enter the invitation code from your medical provider and agree to the release of information.

• If you already have an account, you can add providers to it. Call (888) 670-9775 if you have any questions.

The patient portal invitation you receive after visiting one of our facilities never expires. Once you sign up, you can access the portal anytime, 24/7. Sign in to access the portal online or download the FollowMyHealth app from the Apple App Store or Google Play.

NexGen for heart clinic visits

Our NextGen patient portal gives our heart clinic patients instant access to appointments, test results, refill requests and more—all in one place. It’s the only tool that lets you view most lab, imaging and test results.

Getting started

To create an account, contact the portal coordinator by calling (423) 697-2338. Leave a voicemail with your name, date of birth and email address. Once your account is set up, you’ll receive an invitation to join NextGen at the email you provided during your visit. This invitation will come from noreply@medfusion and include instructions to complete your registration. If you don’t see the email, please check your spam folder.
